Home Health Maintenance
Professional Skilled Nursing Assessment Care / Case Management
As a public health agency, we provide health maintenance visits to citizens in the City of Bismarck and Burleigh County. Clients are accepted into the program based on the reasonable expectation for medical, nursing and social needs to be adequately met in an individual's place of residence. Accepted clients must meet the following requirements prior to being accepted into our program:
- Reside in Burleigh County
- Are under the care of a physician
- Have an identified need that requires nursing services provided in the home to restore, improve or maintain their health
- Have a reasonable expectation that their needs can be safely met in their home
- Do not require in-home nursing as defined by Medicare
- Do not require nursing visits more than once a week
- Able to pay a service fee based on a sliding scale. Medicaid is available for reimbursement of nursing care.
Referrals & Follow-Ups
You may know someone who could benefit from our services. Potential clients may have the following needs prior to being accepted into our program:
- Benefit from nursing visits
- Benefit from medication set-up and medication compliance assessment
- Benefit from an advocate for them with their physicians
